As a parent or guardian, you can access your child's account through your
own LCMC Health Patient Portal account. Below is more information on how
to gain access to your child’s account.

You have an LCMC Patient Portal account.
To gain access to your child's account,

Login to the patient portal.
- Under the Messaging tab, click Request Family Access.
- Complete and submit the form.

You do not have a LCMC Health Patient Portal account.
To gain access to your child's account,
- Access the Request Account Access form

Complete and submit the form. Once approved, you will receive an email
with your activation code and instructions on how to activate your LCMC
Health Patient Portal account. If you fill out the
Child Information section on the form, access to your child’s account may be granted
simultaneously. If you do not or you want to request access to additional
accounts, continue with the following steps.

Login to LCMC Health Patient Portal, and under the
Messaging tab, click
Request Family Access.
- Complete and submit the form.
LCMC Health Patient Portal accounts are granted to patients who are 18
years of age or older.

proxy access can be granted.
How do I get proxy access to my teen’s LCMC Health Patient Portal account?
Your teen, considered age 13 to 17, must approve the proxy access request
privately, in person at his or her next doctor visit.
How do I get proxy access if my child is 18 years or older?
Your adult child, considered 18 and older, must approve the proxy request
privately, in person at his or her next doctor visit.
